These are the sorts of details you will often be asked for when attending hospital and other health-related appointments. Such details include your medical conditions, emergency contacts, operations and known allergies.

It is good to fill in this information and keep it up to date by adding in any new information such as recent procedures
You may also consider asking health professionals you are seeing, to read or take a copy of your health summary information for their records.
